Clinical Nursing Lead for Place-Based Care, Primary and Community Care Services. Cross boundary working within the virtual ward MDT’s. My role involves identification of patients utilising GP electronic risk stratification records, to proactively support the health and well-being of the local population. I provide comprehensive geriatric assessment, case management and anticipatory care planning to individuals who present at risk of deteriorating health or frailty related symptom burden. I first registered as a nurse in 2009 and have had the opportunity to work in a variety of cross boundary primary and secondary services. I am passionate about nursing practice and in particular dementia care with two articles published articles within the area of frailty and person centred dementia care.
